Primary Care APRN Float (Janesville to Rockford/Freeport)
The Primary Care Advanced Practice Registered Nurse, in collaboration with the interdisciplinary team, manages the care of outpatients in the Ambulatory Care Service as a Primary Care Provider as part of a Patient Aligned Care Team. Primary Care is provided in Madison, the West Annex, Rockford, Freeport, Baraboo, Beaver Dam and Janesville. Applicants pending the completion of educational or certification/licensure requirements may be referred and tentatively selected but may not be hired until all requirements are met.
