The Polytrauma/Spasticity Clinic/Commission for Accreditation of Rehab Facilities (CARF) Care Coordinator is a member of the Physical Medicine and Rehabilitation Services (PMRS) under the Rehabilitation and Long Term Care Division (RLTC). The Care Coordinator demonstrates a thoughtful awareness and is committed to a holistic health approach with a focus on personalized, proactive, Veteran-driven care. The location is in Vancouver and will have regular clinic 2-days per week in Portland. Applicants pending the completion of educational or certification/licensure requirements may be referred and tentatively selected but may not be hired until all requirements are met.
