Senior Social Worker- Geriatric Extended Care
The incumbents serve as liaisons between the Saginaw Veterans Affairs Medical Center and the community agencies; advocates for the patients in their care and facilitates resources with the goal of helping patients stabilize within the least restrictive environment possible. These social workers participate in the community care settings and collaborates with Registered Nurses and Advanced Medical Support Assistants staff in this area in community care. Applicants pending the completion of educational or certification/licensure requirements may be referred and tentatively selected but may not be hired until all requirements are met.
