Carilion Clinic is searching for Geriatrics Specialist to join the Section of Palliative and Geriatric Medicine. The candidate will provide inpatient and emergency department observation unit consultation and geriatric emergency department (GeriED) consultative care at Carilion Roanoke Memorial Hospital, telehealth patient care serving other Carilion facilities in southwest Virginia. The candidate may also deliver related follow-up consultative care at the outpatient Carilion Center for Healthy Aging. Carilion Roanoke Memorial Hospital is a 703-bed teaching/tertiary care referral center and one of the largest and busiest Level 1 trauma centers in Virginia.

Responsibilities include:
Evaluate and advise inpatient teams regarding management of older adults in the inpatient setting as a geriatrics consultant regarding complex geriatric syndromes such as delirium, falls, hyper-polypharmacy, high risk medication use, goals of care, multi-morbidity, dementia and caregiver burden.
Evaluate, advise and co-manage older adults in the geriatric emergency observation unit as a geriatrics consultant managing common complex geriatric syndromes such as delirium, falls, hyper-polypharmacy, high risk medication use, goals of care, multi-morbidity, dementia and caregiver burden.
Support emergency department providers in stratification and decision making in the care of patients presenting with geriatric syndromes such as delirium, falls, hyper-polypharmacy, high risk medication use, goals of care, multimorbidity, dementia and caregiver burden.
Support efforts in the emergency department to optimize the care delivered to the geriatric population seeking emergency services.
Support development of policies and practices that enhance the quality of care and patient experience for older adults and their families.
Support ongoing efforts to develop innovative models of care delivery to geriatric aged patients which could include telemedicine and outpatient follow-up clinics
Collaboration and supervision with nurse practitioners and physician assistants on the inpatient and ED based consultation services
Serve as clinical faculty for medical students with Virginia Tech Carilion School of Medicine and Virginia College of Osteopathic Medicine; Carilion Family Medicine and Internal Medicine residents; and Carilion geriatric fellows.
