The Home Based Primary Care Registered Nurse Case Manager (RNCM) a Special Population PACT member, provides direct care and supervises the delivery of quality nursing care to a culturally diverse veteran population of varying complexity in the Veteran's home setting. The RNCM collaborates with VA and community services to effectively meet the health promotion or disease prevention, acute, chronic, and long-term needs, based on the Veteran's goals and plan of care with a focus on self-management.
