Position SummaryThe PPCN Nurse Coordinator is a very unique role within the organization. In this role, you will work independently, and be responsible for outreach to patients, and doctors about the services we can offer for their family’s care. This position is completely remote, and incumbent MUST reside in Arizona. Position requires being onsite a couple of times a month for meetings, training, etc. - NO EXCEPTIONS. We are looking for an experienced nurse, 3 + years in case management role, who has been successful in this type of position, has worked with community agencies, social workers, hospital staff, and providers on cases that can be very simple to the most complex.
