Director - Integrated Care Management

Indiana University Health is seeking individuals who embody these values to join our Fort Wayne leadership team in the role of Director - Integrated Care Management.
The Director of Integrated Care Management provides direction and leadership in care management and coordinates the delivery of health care services. This leader reviews processes and determines performance improvement initiatives and develops and implements care management programs. This Director monitors expenditures related to care management programs and ensures compliance with all state and federal regulations related to care management. This leader collaborates with other leaders in order to ensure efficient and purposeful integration of services for the patient.

Reports to: Vice President- Chief Medical Officer, Fort Wayne
Candidate Qualifications
Candidates for the position will be evaluated according to the following criteria and competencies.
- Requires a Bachelor's Degree in Nursing, Master's Degree in Nursing preferred or requires a Master's Degree in Social Work.
- Requires an active Registered Nurse (RN) license in the state of Indiana or an active Nurse Licensure Compact (NLC) RN license, or requires a Licensed Social Worker, LCSW preferred. If Licensed Social Worker, LCSW must be obtained within 2 years of hire into the position.
- 7-10 years of integrated care management experience required.
- Certification in Case Management preferred.
- Requires strong knowledge of Case Management principles and standard practices.
- Requires knowledge of community resources and appropriate venues for care delivery.
- Requires knowledge of state and national regulatory agency guidelines.
- Requires a strong clinical background with relevant leadership experience supporting patient care transitions.
