AMCR is seeking an experienced Certified Diabetes Care and Education Specialist (CDCES) to provide virtual care services to our large population of patients with diabetes who are utilizing Continuous Glucose Monitors (CGMs), Insulin Pumps, and other devices. Virtual care services to be provided include remote patient monitoring, and principal and chronic care management services, and diabetes self-management education and support (DSMES), among others to patients via telehealth platforms. The person in this role will collaborate closely with our team of endocrinologists, nurse practitioners and other healthcare professionals to improve patient outcomes and quality of life.
The ideal candidate will have significant experience delivering high quality, billable virtual clinical care to people with diabetes, under the supervision of endocrinologists. This role involves delivering patient care, and working with the practice leadership team to build and grow a high quality, comprehensive virtual care program within a redesigned care model. The person in this role will be expected to develop close relationships with clinicians in our Escondido office, while splitting time between working from home and working at our office locations.
Key Responsibilities:
Virtual Care Delivery
Grow and maintain a robust virtual care clinical practice
Cultivate a patient referral process from practice clinicians to the virtual care program
Deliver high quality billable care to eligible patients, including Remote Patient Monitoring, Principal Care Management, Chronic Care Management, Diabetes Self-Management Education and Support
Collaborate with AMCR’s clinical team to create a seamless continuity of care between traditional diabetes care and virtual care services
Develop and implement individualized care plans for diabetes management under the supervision of the practice clinical leadership
Utilize a variety of data sources, including CGMs, pumps, EMR, SDoH, etc. to track patient progress and adjust care plans as needed.
Assist with CGM and pump starts and persistence
Help manage medications, including insulin and other diabetes-related therapies.
Monitor and evaluate the effectiveness of treatment plans, working with practice clinicians to make adjustments as needed.
Documentation:
Maintain accurate and thorough patient records, ensuring compliance with regulatory and billing requirements.
Document patient progress and outcomes in electronic health records.
Quality Improvement:
Participate in and lead quality improvement initiatives to enhance patient care and virtual care operations.
Stay current with advancements in diabetes care, virtual care and participate in continuing education opportunities.
Qualifications:
Minimum of 3 years of clinical experience delivering high quality, virtual care services to people living with Type 1 and Type 2 Diabetes
Successful experience measurably improving clinical outcomes and health status for people living with Type 1 and Type 2 Diabetes, as well as with helping people start and persist with using CGMs, pumps and other diabetes care management tools
Proficiency in using telehealth technologies, EHR systems, and diabetes management software/devices. Experience building a virtual care program a plus
A strong desire to improve care for people with diabetes and healthcare in general
Strong communication and interpersonal skills, with the ability to build rapport with patients and families.
Exceptional organizational skills and attention to detail.
Ability to work independently in a remote environment while maintaining strong collaboration with the interdisciplinary team.
Active California license as a healthcare professional, e.g. RN, RD, PharmD
Certified Diabetes Care and Education Specialist (CDCES) certification required
