Overview
Carle Health is seeking a Nurse Practitioner/Physician Assistant to join our growing Pulmonary Medicine Team and Lung Cancer Team in Urbana, Illinois. This role is a unique blend of clinical practice and patient navigation.
Opportunity Details
Position details include:
Join a team of five Pulmonary physicians and two advanced practice providers
Typical schedule M-F/8am-5pm
Provide healthcare to patients with an emphasis on disease prevention, health promotion, health maintenance, and supportive care
Order, perform, interpret, and act on diagnostic studies
Consult and coordinate multidisciplinary care with other health team members
Provide and discuss treatment options with patient and family
Position includes clinic-based care only
Provides health assessments to include lung cancer screening counseling and shared decision making with patients to determine appropriateness of low-dose computed tomography
Coordinates patient care activities and transitions of care for patients with the interdisciplinary team through a longitudinal care process of lung cancer screening/treatment (includes processes of treatment, scheduling, internal/external referral, and follow-up)
Meets with select patients with lung cancer to present and discuss survivorship plan and follow-up care
Communicates with other health professionals, families and referral sources regarding health status and needs of individual patients
In collaboration with physicians, interprets and evaluates the findings of studies/tests
Assists in the development and maintenance of the Lung Nodule Registry and post-treatment Lung Cancer Survivorship Program
Collaborates with medical leadership to report program metrics in weekly tumor board meetings and optimize workflow for the lung nodule/lung cancer navigator role
Collaborates with medical and administrative leadership to develop care protocols, policies and procedures for the Lung Nodule Clinic and Lung Cancer Navigator role
Educational/Experience Requirements:
Master?s degree in Nursing or Physician Assistant
One year of APP clinical experience is preferred
Consideration for nursing in pulmonary or past Respiratory Therapist experience
Licensure/Certification Requirements:
Current Illinois registered nurse license and advanced practice nurse licensure/physician assistant licensure
ACLS certification

The compensation range for this position is $56.37 - $73.29/hour. This represents a good faith minimum and maximum range for the role at the time of posting by Carle Health. The actual compensation offered a candidate will be dependent on a variety of factors including, but not limited to, the candidate?s experience, qualifications, location, training, licenses, shifts worked and compensation model.
