The Home-Based Primary Care (HBPC) Occupational Therapist (OT) - Clinical Specialist - is located in HBPC Department in the Grand Junction (GRJ) VA Western Colorado Health Care System (WCHCS). The HBPC OT provides care for veterans in their homes with complex - longitudinal - mixed or chronic medical and/or psychiatric conditions - cognitive impairments - and veterans with terminal illness.Qualifications*** This is an OPEN CONTINUOUS ANNOUNCEMENT and will remain open until February 28 - 2025.
